Tara Holley, FNP-BC

Tara Holley is a Family Nurse Practitioner trained in both traditional and functional medicine, blending evidence-based primary care with a holistic, root-cause approach to health. She believes meaningful healing happens when the mind, body, and spirit are treated together—and when patients are truly heard.

As the founder of a Direct Primary Care and Functional Medicine practice in Southern Oregon, Tara provides comprehensive care with the time and depth needed to help patients feel well again. Her goal is to help individuals regain their health, reach meaningful wellness goals, and find answers to the question so many people ask: “Why don’t I feel like myself?”

Many patients come to Tara after being told their lab work is “normal,” despite ongoing symptoms such as fatigue, brain fog, digestive concerns, hormone changes, or chronic pain. Rather than dismissing these concerns, Tara looks deeper to identify underlying imbalances and root causes.

She specializes in treating gut dysbiosis, a common contributor to chronic health issues including autoimmune disease, thyroid dysfunction, diabetes, hormone imbalances, mast cell activation, mental health concerns, leaky gut, and systemic inflammation. Tara partners closely with each patient to create personalized care plans that may include nutrition, lifestyle changes, targeted supplementation, and medications when appropriate.

With over 20 years in healthcare, Tara began her career as a registered nurse before becoming a Family Nurse Practitioner. After years in the traditional healthcare system, she recognized that limited visit times often prevented patients from being fully heard.
